If you cannot attend the annual meeting on October 18, 2026, you can vote by proxy.  Click on the button below to view and print the proxy form.

To vote by proxy, fill out the form, sign it, and do one of the following:

• Choose someone attending the meeting to act as your proxy and have them bring your proxy ballot with them  

• Mail the ballot to the clubhouse at:  LHPOA P.O. Box 22, East Sandwich, MA 02537

• Email the ballot to boardlhpoa@gmail.com. Use the subject header "PROXY BALLOT"

• ​Drop the proxy ballot off at the clubhouse in the BLACK ballot box (it says "BALLOTS" on the front)

Proxy ballots must be received no later than October 17, 2026 to be considered.​
